MediSave for the Chronic Disease Management Programme (CDMP)

Most diseases and medical conditions progress over time. However, it has been found that proper management of these chronic or long-standing conditions can prevent serious complications and reduce the need for costly treatments and hospitalisations. Find a regular family doctor you are comfortable with to confirm the diagnosis and actively manage your health through lifestyle changes and regular check-ups. Timely medical treatment can make the difference.

The Chronic Disease Management Programme (CDMP) was introduced in 2006 and covers 20 conditions:

  • Diabetes Mellitus/Pre-diabetes

  • Hypertension

  • Hyperlipidemia (Lipid Disorders)

  • Asthma

  • Chronic Obstructive Pulmonary Disease (COPD)

  • Chronic Kidney Disease (Nephritis/Nephrosis)

  • Schizophrenia

  • Major Depression

  • Bipolar Disorder

  • Anxiety

  • Stroke

  • Dementia

  • Osteoarthritis

  • Parkinson's Disease

  • Benign Prostatic Hyperplasia

  • Epilepsy

  • Osteoporosis

  • Psoriasis

  • Rheumatoid Arthritis

  • Ischaemic Heart Disease

Under the MediSave 500/700 scheme, CDMP patients can use up to $500 per year to reduce out-of-pocket cash payments for outpatient bills subject to 15% co-payment in cash. For certain more complex conditions, the cap is $700 per year.  There has been no deductible since 1 Jul 2014.

 

Eligible patients can use both their MediSave and CHAS Chronic subsidies for the outpatient treatments of their CDMP conditions at CHAS GP clinics.
